Our spine, as stated by Leonardo da Vinci, is one of the most amazing mechanisms that humans have ever encountered. With 24 vertebrae flawlessly aligned one on top of the other, our spine can bend, flex, turn to the left and right, and do much more. It accomplishes all of this while also safeguarding the delicate and crucial nervous communication system contained within it, the spinal cord. It’s a demanding task.

One of the most common questions we get asked at our spine and pain center is whether physical factors are playing a role in someone’s back pain. Unfortunately, there isn’t always a simple answer to this question. In order to provide the best possible answer, we need to consider a few different things.

In this questionnaire, we will explore if there are physical factors in your life that are contributing to your back pain.

 

Questionnaire - Physical Causes to Back Pain

The following is a list of questions that you may ask yourself to determine if one of the factors causing your pain is PHYSICAL.

Have you suffered one or more serious accidents or physical traumas in the past, even if it was decades ago?(Required)
